Arkansas Licensure Requirements

1. Dental applicants must be graduates of an ADA-approved school.
   
2. Successful completion of the dental exam administered by Southern Regional Testing Agency, Inc. For more information as to dates given, locations, and applications, write:

Note: The Board only recognizes SRTA scores taken within the last five (5) years.


3. Completed State Board application (PDF), with all supporting material including:
   
   • National Board Exam Card (75% minimum score). Copies of the card will not be accepted. Please have the ADA send the card directly to our Board office. Contact National Boards at 1-800-232-1694.
   • Final transcript of grades must bear school seal and be sent directly from school.
   • Copy of Dental Diploma submitted by applicant.
   • Copy of current card for Healthcare Provider level of CPR.
   • A fee (by check or money order) of $150.
     
4. Passage of the Arkansas Jurisprudence Exam which covers the Dental Practice Act, Dental Corporation Act, and the Rules & Regulations. (Study materials and an open-book exam will be sent to applicant upon receipt of application).
