Requirements
Dental hygiene applicants must be graduates of an ADA-approved
school.
Successful completion of the dental hygiene clinical
exam administered by Southern Regional
Testing Agency, Inc. For

Completed State Board application (PDF), with all
supporting material including:
National Board Exam Card (75% minimum
score). Copies of the card
will not be accepted. Please have the ADA send
the
card directly to our Board office. Contact National Boards at 1-800-621-8099.
Final transcripts of grades
must bear school seal and be sent
directly from school.
Copy of Dental Hygiene
Diploma submitted by applicant.
Copy of current card for
Healthcare
Provider level of CPR.
A fee (check
or money order) of $100.
Passage of the Arkansas Jurisprudence
Exam which covers the Dental Practice Act,
Dental Corporation Act, and the Rules & Regulations.
(Study materials and an open-book exam will
be sent to applicant upon receipt of application).
Hygienists must submit proof of completion
of a Board approved course on nitrous oxide analgesia
before the hygienist may
induce and monitor nitrous oxide. Nitrous
courses taken at ADA approved schools are acceptable.
Local anesthesia requires
a separate permit.
